On April 28, 2026, the Office of Childhood received an allegation that the facility has been providing for, and receiving payment for, children who fall outside of their approved age range. After completing an investigation, Compliance Inspector (CI) Jackie Ruth and Compliance Inspection Supervisor (CIS) Marla Chrisco found this allegation to be substantiated based on the following rule violations:
5 CSR 25-500.042(18) which states, "The number and ages of children a group child care home or child care center is authorized to have in care at any one (1) time shall be specified on the license and shall not be exceeded except as permitted within these rules."
AND
5 CSR 25-500.222(10) which states, "All records shall be available in the facility for inspection by the department upon request."
On April 28, 2026, CI Ruth observed the facility's subsidy attendance records for Child A (13-years old), Child B (27-months-old) and Child C (29-months-old). The record for Child A shows that he was counted in attendance from February 2, 2026, to February 27, 2026. The record for Child B shows he was in attendance from February 2, 2026, to April 27, 2026. The record for Child C shows she was in attendance from February 2, 2026, to April 15, 2026.
On May 7, 2026, CI Ruth and CIS Chrisco conducted an unannounced inspection of the facility and held an in-person interview with the assistant director, Courtnie Tomlin. Courtnie said that they cared for school-age children over the summer of 2025, but they no longer watch school-age children. Child A (13-years-old) was enrolled in before and after-care, but he is no longer attending the facility. Child B (27-months-old) is present today, but Child C (29-months-old) is no longer enrolled. She stated that she thought the license started at age 2-years-old. CI Ruth informed her that the facility's approved licensed capacity is from 30-months-old to 5-years-old.
CI Ruth and CIS Chrisco observed a list of children who are currently enrolled at the facility, which included Child B (27-months-old) and Child C (29-months-old). CI Ruth and CIS Chrisco observed the enrollment forms for Child B and Child C, and observed that Child B was present at the facility during the time of the inspection. CI Ruth and CIS Chrisco did not observe an enrollment form for Child A because it could not be located.
After the inspection, CI Ruth attempted to follow-up via phone with the facility director, Yolanda Jones, but no contact was made.
On May 15, 2026, CI Ruth received an email from Yolanda stating that she is dealing with a personal matter and that she would handle things at the facility upon her return.
On May 22, 2026, CI Ruth conducted an unannounced return inspection of the facility and held an in-person interview with Courtnie Tomlin. She stated that Child B is present, and his last day of attendance would be May 29, 2026. CI Ruth observed Child B in care at the facility during the time of the inspection.
On June 5, 2026, CI conducted an unannounced return inspection of the facility and held an in-person interview with Courtnie Tomlin. She stated that Child B is no longer attending the facility, but they are holding his spot and he will re-enroll after he turns 30-months-old. CI Ruth did not observe any children in care at the facility under 30-months-old.
