On May 13, 2026, the Office of Childhood (OOC) received a report alleging on May 12, 2026 in the toddler room, a new staff member was teaching alone for the first time with four children and left a child on the changing table to walk away. The child fell off the table onto her head. Compliance Inspector (CI) JD Greer conducted an investigation and found the allegation to be substantiated based on the following information:
5 CSR 25-500.182(1)(E)3. which states, "The child shall not be left unattended at any time while on the diapering table. "
On May 14, 2026, CI Greer interviewed Director Robyn Bauer, Teacher Jaelyn Barker, and Parent B. Parent B works at the facility. Robyn stated the incident occurred on May 12, 2026 with Teacher Sarah Horn. Sarah was changing Child A and stood her up on the changing table. Sarah briefly turned away, and Child A fell off of the table. Child A had a bruise on her forehead. Parent B stated on May 12, 2026, she had been at the facility most of the day. Approximately 10 minutes after she left the facility, she received a call from Jaelyn. Jaelyn stated that Child A had fallen off of a changing table. Parent B returned to the facility and spoke with Sarah. Sarah stated she wasn't thinking and should have moved Child A to the floor. Sarah stated she knows not to turn away from a child on a changing table; she just wasn't thinking. Jaelyn stated she was in charge of the facility when the incident occurred with Child A and Sarah. A teacher came to her and said Child A had rolled off the changing table and Sarah needed help. Jaelyn had another teacher step into the classroom for Sarah. Jaelyn observed a bruise on Child A's forehead. She watched the video of Sarah with Child A. She observed on camera Sarah stood Child A up on the changing table. Sarah briefly turned away from Child A, and Child A fell off of the changing table.
On May 14, 2026, CI Greer reviewed an incident report from May 12, 2026, and observed that Sarah Horn was present with Child A. Child A was being changed and fell off the changing table and bonked her head.
On May 14, 2026, CI Greer reviewed video footage of the incident from May 12, 2026 and observed Teacher Sarah Horn present wearing a dark short sleeved shirt. Child A is lying down on the changing table. Sarah stands Child A up on the changing table and pulls her shorts up. Sarah turns to her left to assist a child, and Child A falls forward off of the changing table.
