On May 28, 2026, the Office of Childhood (OOC) received a report alleging that there is a foot-sized hole in the wall by the stairs walking in. After conducting the investigation, Compliance Inspector (CI) Curtis West finds the allegation is substantiated. This conclusion is based on the following evidence of a licensing rule violation that occurred at the facility:
5 CSR 25-500.082(2)(A)6., which states "Walls, ceilings and floors shall be finished with material which can be cleaned easily and shall be free of splinters, cracks and chipping paint. Floor covering shall be in good condition. Lead-free paint shall be used for all painted surfaces."
On May 29, 2026, CI West conducted an unannounced inspection and interviewed Director Connie Mayfield, Caregiver Carol Harkless, and Caregiver Morgan Coon. Connie stated that the hole in the wall has been there for at least a year.
Carol stated that she had noticed the foot-sized hole in the wall close to the floor when she first walked in the door. Morgan stated that she had seen the hole in the wall.
On May 29, 2026, CI West conducted a walkthrough of the facility and observed a foot-sized hole in the wall by the stairs walking in. The outline of the hole was uneven and torn in areas. There is something that appears to be insulation showing, as well as something black.
