| Conclusion Summary |

On June 5, 2026, the Office of Childhood (OOC) received a report alleging there is standing water at the southeast corner of the head start playground after it rains. It's been reported to the company for years. After conducting the investigation, Compliance Inspector (CI) Rebecca Muckelrath finds the allegation to be substantiated. This conclusion is based on the following evidence of licensing rule violations which occurred at the facility:

On June 10, 2026, CI Muckelrath interviewed Director Roxine Mallory and Reporter and Teacher Scott Anzalone. Roxine stated she has received reports of standing water on the playground that the bigger kids use. Scott stated he has marked the standing water issue on the Playground Safety Checklist.
On June 10, 2026, Compliance Inspector (CI) Rebecca Muckelrath conducted an unannounced walkthrough of the facility and observed standing water on the Southeast corner of the playground used by the bigger kids.

The facility shall notify all caregivers (paid employees and paid or unpaid volunteers) of the violations which were substantiated and specify the actions to be taken by all caregivers, in order to comply with all violations cited. A copy of the memo, letter, or meeting agenda shall be submitted to the Office of Childhood.
