On June 10, 2026, the Office of Childhood (OOC) received a report alleging that Parent A has observed a section of damaged flooring in the infant room that has been covered with interlocking foam mats. This condition has been present since at least May 12, 2026. After conducting the investigation, Compliance Inspector (CI) Curtis West finds the allegation is substantiated. This conclusion is based on the following evidence of a licensing rule violation that occurred at the facility:
5 CSR 25-500.082(2)(A)6., which states "Walls, ceilings and floors shall be finished with material which can be cleaned easily and shall be free of splinters, cracks and chipping paint. Floor covering shall be in good condition. Lead-free paint shall be used for all painted surfaces."
On June 12, 2026, CI West conducted an unannounced inspection and interviewed Director Jessica Bailey and Caregiver Breanna Spencer. Jessica stated that she is aware of the missing tiles on the floor in the infant classroom. She is aware that the edges of the tiles could cause harm to the children. There was some remodeling done, and she has ordered the tiles to replace the ones missing on the floor. Breanna stated that she knew there were a few tiles missing on a section of the floor inside the infant classroom. She agrees that the edges of the missing tiles could be hazardous to the children.
On June 12, 2026, CI West observed a section of damaged flooring in the infant room. The edges of the missing tiles could be hazardous to crawling infants.
On June 12, 2026, CI West interviewed Parent A, who stated that she noticed that they are covering a section of the floor that has some missing tiles with a mat.
